Sub-acute rehabilitation care provided in skilled nursing facilities offers in-depth medical care combined with rehabilitation services so patients who are well enough to be discharged from the hospital, but not yet fully able to care for themselves, are paired with trained providers who deliver compassionate, expert care to help residents achieve their highest potential. The sub-acute floor at the Hebrew Home, staffed by Montefiore physicians, opened on October 1, and will accept referrals from all acute hospitals, as appropriate.
