UNIVERSITY-BASED VERSUS COMMUNITY-BASED ACUTE CARE

A one-year study of 118 patients admitted to a long-term acute care facility showed distinct differences in critical care treatment among those whose care was directed by either university-based or community-based physicians. Patients of university-based physicians were more likely to have a shorter hospital stay, to be more quickly and successfully liberated from mechanical ventilation, to have life-sustaining therapy withdrawn more often as a result of critical illness, and, when dying, to be less likely to receive cardiopulmonary resuscitation. Also, the reimbursement to the university-based practitioners was about half that of community-based physicians. This research appears in the November issue of the American Journal of Respiratory and Critical Care Medicine..

REDUCING OCCUPATIONAL ASTHMA AMONG BAKERY WORKERS

Dutch researchers found that 90 bakery workers (23%), out of 393 employees, reported work-related inflammation of the mucus membranes lining their nose (rhinitis) and chest tightness. The investigators recommended that bakery managers reduce the workers' exposure to wheat proteins in the bakery dust (wheat allergens) to that of a normal population. This would require a six-fold reduction for highly exposed dough makers and bread bakers. Their research appears in the November issue of the American Journal of Respiratory and Critical Care Medicine.

RESPIRATORY HAZARDS FOR AGRICULTURAL WORKERS

A major supplementary report, endorsed by the American Thoracic Society and the National Institute of Occupational Safety and Health, addresses many aspects of respiratory illness among varied groups of agricultural workers. It covers the problem worldwide; the exposure, pathology, and clinical features of various respiratory diseases; the natural history of the illnesses; and the areas needed for future research. This report contains extensive new information directed at the identification and prevention of respiratory diseases in the agricultural workplace as gathered by over 30 experts. It is a supplement to the November issue of the American Journal of Respiratory and Critical Care Medicine.
